Billing Information

Unfortunately, Dr. Tomaso does not accept insurance. However, you will be provided with a superbill that outlines the services completed and relevant billing codes for the insurance company’s review.

Note that the insurance company may or may not reimburse you for some of the cost, and/or they may apply the costs to your deductible.

Evaluation fees are as follows:

• $150.00 per hour for the intake and feedback sessions

• $250.00 per hour for direct assessment

• $200.00 per hour for planning, preparing, scoring, analyzing, and report writing

A good faith estimate of the evaluation costs will also be made available before starting the evaluation process.
